WebSMARTT Cancellation or Deletion of Product Being Delivered
In order to cancel or delete items for delivery, date submitting this form must be 3 working days prior to delivery date. Example: On Friday, 5/22 cancel items for delivery Thursday 5/28 because of Monday being a holiday. Complete/submit a form for each different delivery date.
